HEART RESERVE

HOSPITALS AND HEALTH PLANS

Care should continue
after she goes home.

Postpartum Advocate connects the discharge plan, patient understanding and completed follow-up—with a responsible service for every unresolved concern.

Proposed hospital-branded service · Fictional demonstration · Clinical delivery and funding are not activated.

ONE PLATFORM. ONE CLEAR FIRST OFFER.

HEART RESERVE Postpartum Advocate

Start with pregnancy-related hypertension and preeclampsia, after vaginal or cesarean birth. HEART RESERVE Concierge provides the shared patient experience. Surgical Recovery Concierge is a separate second white-label offer with its own population and clinical scope. The continuing Heart Story connects recovery to future visits, heart health, midlife and separately scoped metabolic care.

The proposed fee is per activated episode, with setup and evaluation priced separately. The goal is to earn renewal by demonstrating completed care, safe operations and acceptable total cost against the purchaser’s current service.

01 / PROGRAM AGREEMENT

The member and care team experience

Each member should have an understandable next step and a reachable service responsible for unfinished care. The following is the proposed production scope; prototype interactions demonstrate the workflow only.

StageRequired behavior
Before dischargeVerify patient, plan source/version, medicines, due assessments, contact hours, consent, access needs and two-way contact
Today at homeShow the approved plan, a brief explanation, a symptom-help route and one manageable next action
When a barrier appearsRecord the issue, time, responsible team, due window and next action; preserve the patient’s own words
Until the outcome is knownSeparate recording, delivery, acknowledgement, assessment, patient contact and resolution; allow reopening
End of the episodeCarry unresolved actions forward and record receiving-service acceptance, contact route and next appointment or contact date

Upgrades included in the combined demonstration

The main hospital dashboard now adds a source-and-version discharge review record, structured closure evidence, a missed-check-in scenario, an outage state that blocks simulated queue updates, and a receiving-service acceptance record. Editing the plan or handoff clears its prior simulated confirmation. A rehearsal export retains the fictional events.

Patient advocacy

Keep “I am still concerned” available; preserve repeated concerns and unresolved medication discrepancies. A normal-looking home reading must not dismiss concerning symptoms. Teach-back records what the patient understood and whether human review occurred. Missing information remains unknown. Caregiver access uses selected fields and revocable permission; downloaded copies cannot be recalled.

Continuity beyond recovery

Retain pregnancy history, dated original results and questions for ongoing cardiovascular care. Offer recovery-appropriate Life’s Essential 8 education under the care plan. A 90-day funded episode does not provide a full year of clinical coverage; the receiving service and subsequent funding must be explicit.

02 / PROGRAM AGREEMENT

Clinical delivery and production acceptance

PartyProposed responsibility
HEART RESERVEOriginal curriculum, Heart Story structure, advocacy and follow-through workflows, staff preparation and measurement definitions
Technology partnerDemonstrated platform, configuration, reliable communication, access controls, integrations, hosting and technical support
Hospital or contracted clinical groupAssessment, prescribing, clinical review, staffed response, backup coverage and continuing care
Health plan and evaluatorBudget ownership, current-service baseline, approved data access, evaluation and renewal decision

Clinical ownership

Before enrollment, the clinical partner must approve eligibility, assessment deadlines, escalation rules, response hours, named primary and backup services, unreachable-patient procedures and emergency instructions. Medication changes and clinical assessment stay with the licensed care team. Clot-prevention reminders appear only when prescribed. AI availability must not imply continuous human coverage.

Acceptance evidence required from the platform

Human review of automated content

Answers must use approved, versioned material and the correct patient context. Preserve source provenance and uncertainty. Evaluate unsupported statements and unsafe reassurance before use. Photo extraction, ECG interpretation and additional device connections require separate specifications and any applicable licensing or regulatory review.

Buying decision

Use a paid scoping phase to confirm the current-service gap, eligible discharge volume, platform and clinical quotes, data access and renewal criteria. Select the prime contractor based on purchaser requirements and delivery capacity. Emagine is a proposed platform and licensing partner; its role is not represented as contracted.

03 / PROGRAM AGREEMENT

Pilot evaluation and financial plan

Up to 100 total evaluation participants: a planning assumption of 50 program participants and 50 concurrent current-care comparison participants. Both groups retain clinically indicated care. Allocation, consent and oversight require approval before enrollment.

The primary purchasing question compares the full proposed service with the care and maternity benefits members actually receive today. A separate Journey-with-versus-without-HEART-RESERVE comparison could isolate the program contribution; it is not a substitute for the payer comparison.

PeriodWork
Months 1 to 3Contracting, configuration, content review, access testing and staff rehearsal
Months 4 to 8Staged recruitment, beginning with an initial operational review group
Months 9 to 11Finish the last 90-day episodes and accepted continuing-care handoffs
Month 12Operational analysis and decision about the next stage
After agreed claims runoutComplete the financial analysis when claims are sufficiently mature
MeasureDefinition
PrimaryClinician-specified early BP evaluation completed by its due date; distinguish reading submission from clinician review
Follow-throughMedication access, appointment barriers, due actions and accepted handoffs with documented outcomes
Understanding and accessTeach-back assessment, unassessed responses, offered/enrolled/activated counts and participation by language and channel
Safety and workloadMissed urgent concerns, delivery failures, unresolved task age, adverse events and staff minutes
Exploratory financial outcomes30- and 90-day ED visits, readmissions and total maternal paid claims across facilities, plus full incremental program costs

Analyze all assigned participants where appropriate and report activation separately. Prespecify denominators, missing records, enrollment changes and adjustment. Completion, safety and workable staff effort are the first decisions. A feasibility sample cannot establish mortality reduction or reliable savings from uncommon events.

Earlier pilot budget — superseded

View the itemized budget, illness/readmission evidence and hospital value worksheet →

The illustrative development, delivery and evaluation envelope is $150,000 for 50 service recipients plus 50 comparison participants. The companion funding plan itemizes it. That is $1,500 per evaluation participant or $3,000 per service recipient when the entire pilot budget is divided that way; neither figure is a recurring episode price.

Recurring cost must be quoted as platform, HEART RESERVE license, clinical time and coverage, devices, access support, reporting and allocated operating overhead. Keep first-time development and research separate. Net payer value equals adjusted current-care maternal paid claims minus program-group maternal paid claims minus additional program costs not already in claims. Include additional needed care and outside-hospital events. Hospital avoidable expense and revenue effects require a separate calculation.

04 / PROGRAM AGREEMENT

Licensing and the next development stages

Negotiate a fee per activated 90-day episode or an agreed minimum license with usage reporting. Activation requires eligibility and consent, a reviewed plan, an assigned clinical service and successful two-way contact, with device setup when required. Define duplicates, cancellations, nonactivated outreach, extensions, payment dates and audit rights. No fee is triggered merely by a symptom report or AI message.

HEART RESERVE should retain ownership of its existing content and brand. Agree rights to new work, permitted editing, hospital branding, sublicensing, data portability, support, termination and continuing care at exit. The existing LLC’s exact legal name, vendor documentation and authority must be verified before contract execution. These are negotiation points, not agreed legal terms.

Months 1 to 3Approved content and workflow, configuration, permissions, failure-case rehearsal and enrollment-readiness decision
Months 4 to 12The focused feasibility pilot and operational report, subject to funding and approvals
Months 13 to 18 if justifiedClaims analysis after runout; refine the service and design a statistically planned larger evaluation
Months 19 to 24 if fundedSeek a renewal or broader evaluation contract; add a second site or pathway only after readiness and staffing review

Second white-label offer

Surgical Recovery Concierge carries the same patient advocacy, plan comprehension and documented follow-through into a separately defined surgical pathway. Its procedure, eligibility, episode length, staffed response and full price must be agreed before enrollment. It does not inherit the postpartum pilot budget or clinical rules.

Expansion options

Priority expansion is continuing pregnancy-to-heart care. Midlife and hormones, Weight Strength and GLP-1, general surgical recovery, report extraction and licensed ECG analysis remain separately scoped options. Each needs a defined population, clinical pathway, price and evaluation. Existing demonstration screens do not establish production availability.

Sources and market context

CDC identifies urgent maternal warning signs during pregnancy and the year after delivery; this supports a continuing-care handoff, not a 90-day mortality-prevention claim. AHRQ teach-back guidance informs the understanding workflow. AHA Life’s Essential 8 informs lifestyle education. Existing Aria, Pomelo, SeamlessMD and Megi offerings mean AI, education and pregnancy-to-heart positioning alone are not exclusive.
