ONE PROGRAM · ONE HOSPITAL
| Patient and program | One hospital; adults after pregnancy-related hypertension or preeclampsia, following vaginal or cesarean birth; a 90-day service episode. |
|---|---|
| Recovery plan and Heart Story | Use the verified discharge plan, relevant pregnancy history, medicines, follow-up appointments, questions and care-team contact instructions. Record source and review status. |
| Teaching and check-ins | Use a focused, clinician-approved content set with symptom check-ins, medication-access and appointment support, and a simple check of understanding. |
| Patient advocacy | Support “I cannot do this” and “I am still concerned.” Preserve the concern, responsible team, due time, delivery/review status and outcome; allow an unresolved concern to be reopened. |
| Bluetooth BP | One exact supported upper-arm cuff, appropriate validation evidence for pregnancy/preeclampsia, suitable cuff sizes and compatible phones. Show a reading reaching the correct patient’s staff record. |
| Continuing care | Record the receiving service, accepted handoff, next contact and unresolved needs. State what record access and education remain after the paid episode ends. |
| Care delivery | Hospital or separately contracted licensed team owns assessment, treatment, response windows and backup coverage. AI availability is not continuous human monitoring. |
Later options: custom EHR development, ECG or laboratory interpretation, GLP-1 and midlife modules, additional monitored pathways and formal comparison-group research. If an item is necessary for the first release, identify and quote it rather than silently exclude it.
VENDOR RESPONSE SCHEDULE
For each line, request an amount or a clear “included,” “excluded” or “not available.” Ask the partner to identify the current product release and any assumptions. Blank fields are unresolved, not zero cost.
| Scoping and connector demonstration | Fixed amount; deliverables; whether this is credited toward configuration. |
|---|---|
| White-label setup and content configuration | Fixed amount and included revisions; state which requested functions exist today, need configuration, need development or are unavailable. |
| Cuff connection and equipment | Exact cuff model; connector/API provider; supported phones; setup charge; per-device and recurring fees; shipping, sizes, replacements and patient support. |
| Platform, AI and technical support | Monthly and/or per-active-patient charges; definition of “active”; usage caps and overages; implementation-period fees; minimum term and minimum spend. |
| Clinical service and onboarding | Included/excluded tasks, staffing and coverage hours; hospital obligations; price any supplied clinical staff separately from software support. |
| 50-patient, 90-day service | Total for all 50 completed episodes; assumptions about enrollment timing, operating months and inactive participants. First 5–10 belong within the 50, not a second cohort. |
| Rights and future licensing | Who owns existing and newly created content/code; permitted hospital branding and sublicensing/resale; who contracts and invoices; whether an episode-based HEART RESERVE program license is permitted; exit/export terms. |
| Delivery and total commitment | Timeline, dependencies, acceptance criteria, payment milestones, quote validity, taxes, annual commitments, renewals, change orders and all exclusions. |
Keep cost and revenue distinct. A platform quote establishes supplier costs. HEART RESERVE’s hospital price, margin and any founder royalty require a separate commercial agreement. Per-episode licensing is a proposed structure to discuss, not a right already granted.
EVIDENCE BEFORE CLINICAL ACTIVATION
Use fictional records for development and rehearsals. Live enrollment begins only under the agreed clinical service and approved data arrangements. The first 5–10 participants are an operational checkpoint within the 50-person service cohort.
AFTER THE ITEMIZED RESPONSE
The current $35,000–$60,000 setup range remains an internal planning model. The complete 24-month proposal includes a broader pilot and evaluation. No savings, insurance payment or grant award is assumed.
